Department of Disability (DDD) Approved Vendor Services
Our Services
We are contracted with Arizona Department of Disabilities (DDD) and only onboard members who have qualified for Arizona Long Term Care (ALTC) services via DDD. In addition, we certify parents to become caregivers for their minor and adult children or their loved ones at no cost. Our scope of services are Habilitation, Attendant Care, Respite, and Home Maker.
Habilitation (HAB)
Habilitation is a service that provides methods and training to increase a member’s ability to become more independent and improve upon life skills.
These services are designed based on the members need. Below are some example habilitation services:
- Independence and socialization skills.
- Safety and community skills.
- Member’s health and safety.
- Essential activities required to meet the Member’s personal and physical needs.
- Alternative and/or adaptive communication skills
- Self-help/living skills.
- Developing skills for independent living in their home, including adaptive and self-determination skills, while offering supervision and assistance to assure their health and safety etc.
Attendant Care (ATC)
Attendant Care is a service that provides a member with assistance for personal care tasks, home tasks, or specialized supervision.
Some example services:
- Meal preparation and clean-up (e.g., meal planning, food preparation, cooking, storing food, cleaning the dishes).
- Eating and assistance with eating (e.g., prompts to eat slowly, proper positioning while eating, monitoring for choking).
- Bathing (e.g., transferring into the tub or shower, adjusting water temperature for safety, monitoring for drowning risk, use of assistive devices).
- Dressing and grooming (e.g., oral hygiene, nail care, shaving, hair styling, putting on assistive devices).
- Toileting (e.g., bowel and bladder care) etc.
Respite (Res.)
Respite is a service that provides relief to parents, primary caregivers, and natural supports with short term care. Some example services:
- Respite caregivers can help with essential tasks like bathing, dressing, using the restroom, and eating.
- Light housekeeping: Respite caregivers can help with light housekeeping tasks like laundry and dusting.
- Basic medical care: Respite caregivers can help with basic medical care like checking on wounds and helping with medications etc.
Home Maker
A service that provides a member with assistance with daily home making services.
Some example services:
- Provides relief to parents, primary caregivers, and natural supports with short term care.
- Laundry: Homemakers can wash, dry, and fold laundry, and sometimes iron.
- Shopping: Homemakers can shop for and store household supplies and medicines.
- Personal care: Homemakers can help with bathing, dressing, toileting, and feeding etc.
